What is Motilium?

Stomach heaviness, epigastric fullness, and early satiety are many shades of one of the common stomach diseases. Stomach heaviness is often accompanied by sickness, bloating, heartburn, and dull pain in the stomach’s pit. These complaints are usually caused by gastric motor disorders that cause unpleasant or painful feelings in the upper abdomen, which, in turn, interfere with daily activities, restrict the diet, movement, rest, and physical activity and worsen the quality of the patients’ life. According to the statistics, up to 45% of the citizens of the world (regardless of their social and financial status) experienced stomach heaviness; however, according to studies, only 20–25% of patients seek medical help, preferring to follow the pharmacist’s recommendations.

Enzyme preparations are intended for splitting food in the intestine; therefore, they act under alkaline conditions (which is normal for the intestines). The acidic environment of the stomach destroys them because, by their chemical nature, all enzymes are proteins. Therefore, all enzyme preparations are covered with a special protective acid-proof (enteric-soluble) shell, due to which they act only in the intestines.

Normal gastric motility is provided through the muscle layer’s coordinated work, while food is chopped, mixed with gastric juice, and moved in the distal direction. The propulsive stomach activity and the condition of sphincters are regulated by the autonomic nervous system and gastrointestinal hormone. Violation of the stomach and humoral factors’ nervous regulation leads to the disruption of the coordinated work of the muscle layer and sphincters. This, in turn, causes hyper- or hypotension of the stomach and motor disorders (slow digestion or acid reflux). Delayed movement of the stomach contents in the distal direction causes complaints of stomach heaviness, epigastric fullness, and early satiety.

Considering the mechanisms of occurrence of such symptoms as stomach heaviness, epigastric fullness and early satiety, the use of prokinetics is justified in order to improve gastric motility and eliminate of these manifestations. Today, one of the safest and most effective of them is Motilium.

Indications

Motilium is used to eliminate the states caused by gastroesophageal reflux disease, delayed gastric emptying, and esophagitis, accompanied by bloating, abdominal fullness, vomiting, epigastric pain, sickness, heartburn, regurgitation, and flatulence.

In addition, the prescription of Motilium is advisable in the following cases:

• sickness and vomiting, associated with the use of dopamine agonists during the treatment of Parkinson’s disease;
• sickness and vomiting of an organic, infectious or functional origin;
• sickness and vomiting, caused by eating disorders, intake of medications and radiotherapy;
• cyclic vomiting syndrome, rumination syndrome, gastroesophageal reflux, as well as other gastric motor disorders in children.

Contraindications

Motilium is not prescribed to the patients with:

• perforation or intestinal obstruction;
• tumor lesion of the pituitary gland, secreting prolactin;
• gastrointestinal bleeding;
• hypersensitivity to domperidone.

Joint prescription of Motilium and Ketoconazole, produced in oral dosage forms, is unacceptable. Because of the fast metabolism of domperidone in the liver, the drug should be prescribed with special caution to the patients who have severe violations in the work of this organ.

Use during pregnancy and breastfeeding

Due to a lack of data on the influence of domperidone on the development of the fetus and breastfed babies in mothers taking Motilium, the drug isn’t used in the treatment of pregnant and lactating women.

Way of use and dosages

It is recommended to take Motilium 15 or 30 minutes before eating and (if necessary) before going to bed.

To decrease the symptoms of chronic dyspeptic disorders, adults are prescribed 10 mg (or 1 pill) 3 times a day. If the use of the starting dose doesn’t yield the necessary therapeutic effect, a single drug dose can be increased to 20 mg or 2 pills. The maximum daily dose is 80 mg.

For the treatment of sickness and vomiting, patients are prescribed 20 mg of the drug 3-4 times a day.

Motilium in pills can be used only in the treatment of adults and children, whose body weight exceeds 35 kg. In pediatric practice, it is recommended to use the drug in the form of suspension.

Overdose

In the case of an acute overdose of Motilium, the patient can have increased drowsiness, spatial disorientation, and extrapyramidal symptoms. To treat the overdose symptoms, the patient is given a gastric lavage and prescribed a corresponding symptomatic treatment.

Side effects

The use of Motilium can be accompanied by endocrine disruptions, development of extrapyramidal symptoms, gastrointestinal disorders, spastic colon, and allergic reactions.
